Please bring the following to your appointment:
- Insurance card
- Driver's License or valid ID
- Copies of images (X-rays, MRI, CT scan)
- Copies of radiology reports (X-rays, MRI, CT scan)
- Copies of external records
- Physician referral if required by insurance
- List of medications
Payment
Copays are due at the time of service. Our office will run benefits, but we ask that you call the number on your insurance card to verify benefits. We will bill your insurance company if you have insurance covering your visit.
Self - Pay - Our office does provide self-pay pricing. You can pay for a package up front with no hidden cost.
